Enrollment
If you have been offered a placement (and we have many places available) with OOSH NORTHERN BEACHES you will need to complete your child/children's enrolment.
Ensure you indicate what centre your child will be attending.
*Please complete a Registration Form. (only one per year)
You need to complete all four pages so that we have all information on your family including parent, child and emergency details. This is very important!
*Please complete a Booking Form (please see booking form information)
This two page form indicates what days your child will be needing care and the methods of payment. You must indicate to us what centre your child attends and circle the days they are attending. A booking form can be completed prior to your child attending weekly, monthly, termly or yearly if you are aware of your regular days. We prefer direct debit as a method of payment and this form should also be completed.
For Casual Bookings
A booking form must be filled out when your child attends our centre on a casual basis. All forms must be completed prior to your child's first booking but after that date, with a method of payment chosen from the four options on the back.
If you are unable to fill out a booking form each please notify the centre (on the centre phone contact) of the day when your child will attend. A staff member will confirm they have received your booking.
